update for ANSI and VS2008 build modes
* VS2008 now compiles with SSE2 support.
* Make sure that the ANSI fallback interprets 16-bit elements.
* Fix warnings about truncating 16-bit zero-extended hex to s16's.
0x8000 to 0xFFFF got truncated to s16; we just want a signed offset.
* Merge pull request #68 from cxd4/can_haz_uber_l33t_ADPCM
Buffer ADPCM 128 bits at a time, rather than 16 bits at a time.
